In order to apply, the individual must have already applied for and be approved for admission to Evoke Therapy Programs.  You may request the Evoke Family Foundation grant application from your Evoke admission representative.

Step 1: Apply for Admission at Evoke Therapy Programs.

Step 2: Once approved for admission, download the attached application and return to director@evokefamilyfoundation.org OR complete online application.
